As the Church grew the Apostles began appointing elders. The answer is, I believe, eldership - humble servant leadership.

Eldership only works if the Pastor is truly willing to be subject to the elders and the elders are willing to discipline a wayward Pastor, even to the point of removing a popular Pastor from the pulpit. In many cases churches with these dynamic, popular Pastors don't have that needed situation - the elders are almost symbolic. And the Pastors at churches like that aren't going to be subject to their peers either, largely because they often view other churches as competition, not teammates in the great commission. Oh, their peers may argue theological issues publicly, but they won't criticize behavior, because they know the skeletons in their own closet.
