Here at SouthCross Community Church, we have a Youth Group Missions Prayer Team. I have yet to go, but from my understanding it is basically praying for the youth going to Mexico this summer. We use the Holy Bible as our resource and also most likely will use Prayer Transformation Ministries Prayer Guide.
Our church has link missionaries in several places around the world, and usually as part of our weekly prayer bulletin one of the families/couples/people will be profiled with updated news and things that we can pray for. Resource wise, the church (and congregation members who have asked to receive correspondence) receives newsletters via email and prints copies for the congregation. These will have updates as to what's been happening since the last newsletter and prayer requests. We used to have bi-monthly prayer nights on a Tuesday where misison prayer would be part of the night, but unfortunately this hasn't worked as well as hoped. However all the bible study groupls within church incorporate a solid prayer time into their meetings
We have people who are trained to pray for others. This usually takes place in the Pastor's study after the morning service. It takes place in the study to respect the confidentiality of those seeking prayer. There is usually a different prayer team every Sunday.
