Perhaps this will clarify for all of you what the traditional Catholic teaching on death is.
I had to shrink this chart down. If you want the original-size copy, PM me and I will get it off to you.

What's wrong with it? Certainly it's blunt, but is that such a bad thing?

It is blunt. Sometimes people need a bit of that.

There are a few errors:
Tartarus isn't the same as Hades. Hades can mean either Sheol as a whole or the "negative half." Tartarus in reality is a holding bin if it were of the Devil's military of fallen angels.
It is possible that some non-Christians might end up in Paradise
Otherwise, that's extremely accurate. I give it a 95% (A)! :)

If this is the traditionally Catholic teaching, why is it that neither the Roman Catholics or the Eastern Orthodox teach it? Is the truth on such an important issues embodied solely in the teachings of handful of bishops? Anglicanism is small compared to Roman Catholicism and Eastern Orthodoxy, and the bulk of Anglican bishops seem to believe in a simpler heaven or hell scenario. Even amongst Anglo-Catholics, some subscribe to the Roman Catholic or Eastern Orthodox view rather than the scenario you paint. It seems farfetched to think that 99% of the body of Christ, and maybe 97% of the apostolic body of Christ could have lost sight of such a crucial bit of revealed truth.

The EOs do teach this.

Hmmm, not according to some folks I've spoken to, but they weren't EO and thus wouldn't have firsthand knowledge. I'll ask over on the EO board. :)

Yes you are right. I noticed that, but didn't really try to change it or anything. I'm not sure why the author of this chart used "tartarus" to describe the place, when Jude makes it very clear that Tartarus is where the fallen angels are being kept.

Well, everything I have read as official statements of Orthodox belief have taught this. Every orthodox I have spoken with has taught this.
Is it possible that different Orthodox jursidictions hold to different theologies regarding this?
Either way, this is the earliest and most consistent viewpoint we see among the fathers of the Church.

Photoshoping someone into Purgatory is out of line - or teasing Naomi?
Naomi, I am convinced you will be in heaven one day.

It just seemed a strange time to inject humour, I guess.
