If you start a thread meaning it to be inoffensive, are you at fault if a flame war breaks out on it between other posters?
This happened to me once when a thread I had started about Hal Lindsey turned into "Preterist-Futurist Armageddon." The thread was closed but I'm afraid to begin any discussions about prophecy again lest I be banned for the conduct of others replying to me.

Just because a thread was closed does not mean that it was your fault it was closed.
Generally threads are closed because discussions that have evolved are either getting way out of hand (ie flame fest) or are just repeating the same things over and over and over again without any new ideas.
So, to answer your first question...no you are not at fault if you start a topic and then a flame war breaks out. Now, if you were part of the flame war, that's another story. ;)

Susan, as explained to you in the past, a person is banned because he/she has violated the rules. A member does not get banned because others break the rules. Each member is held accountable for his own actions, not the actions of others. I hope this helps to better understand its definition. :)
